Penerapan Metode Bowtie dan SCAT untuk Analisis Kecelakaan Dump Truck Terguling di Pertambangan Batubara

The company under study recorded 25 property damage cases involving dump trucks on the hauling road during 2023–2024, with 8 cases resulting in dump truck rollovers. This study analyzed 2 property damage cases using the Barrier-Based SCAT method, which combines the Bowtie method and the Systematic Cause Analysis Technique (SCAT). The analysis results showed that the top event, namely dump truck rollover, was influenced by two main threats: operator failure in operating the unit and operator microsleep. Of the 14 analyzed barriers, 35.7% were considered effective, while 64.3% were ineffective. The SCAT analysis indicated that barrier failures were dominated by unsafe acts at 77.8% and unsafe conditions at 22.2%, while all basic causes were dominated by job factors. The proposed control recommendations focused on strengthening administrative controls by 72.7% and engineering controls by 27.3% through improvements in training systems, procedure implementation, fatigue management, operational supervision, and emergency preparedness to prevent similar accidents in the future.
